Assumption of Risk and Release from Liability ("Agreement")

I, the undersigned, give permission for my Child to participate in the STEM Summer Camp, offered by the School of Science at Indiana University Indianapolis, on behalf of The Board of Trustees of Indiana University (“IU”), taking place between June 8, 2026, and June 11, 2026 (“Program”).

In consideration for my Child’s participation, I, individually and on behalf of my Child, agree to the following:

I understand activities for the Program may include, but are not limited to, the following: physical activities (e.g., running, jumping, climbing); physical exertion such as lifting or moving heavy objects; spending extended periods of time outdoors being exposed to the elements (sun, wind, rain); consumption of food and/or beverage; and the following additional activities: participating in activities from all eight programs and departments in the School of Science (e.g., dissections; using chemicals); being in laboratories; being outdoors; eating in the dining hall; touring the IU Indianapolis campus; speaking to current students; building rockets; and going off-campus to a local (i.e., within walking distance of campus) zoo or museum.
I understand that as part of my Child’s participation in the Program there are dangers, hazards, and inherent risks to which my Child may be exposed, including, but not limited to, such things as incidents related to the above mentioned activities, including sprains, broken bones, cuts, bruises, entrapment, temporary or permanent disability, and/or death; adverse weather conditions; exposure to theft and other criminal activity; allergic reactions to food and drink items; damage to clothing or personal items; other physical, mental, and emotional injury; fire or other emergencies; other risks and dangers, whether known or unknown nor reasonably foreseeable; and the following additional risks: emotional distress; allergic reactions caused by exposure to animals or environment; harm caused animal escape or contact with animals (e.g., bites, scratches); exposure to sharp objects; burns; chemical exposure; allergic reactions to equipment or fumes; risks associated with equipment use or misuse; equipment failure or malfunction; exposure to illness; food poisoning; sunburn; dehydration; insect bites; environmental hazards; physical exertion; exercise-induced health issues; and interactions with other participants, staff, or volunteers.
I fully understand the scope of the activities and the risks involved. I voluntarily accept and assume all risks of injury, loss of life, or damage to property arising out of my Child’s participation in the Program.
I hereby release and fully discharge IU, including its officers, employees, and agents, from any and all claims or causes of action, including all liability for damage to personal property or personal injury which may result from my Child’s participation in the Program, that may be brought by me or my Child or for any injury or loss that my Child may suffer while participating in the Event to the fullest extent permitted by law.
I further release, indemnify, and hold harmless IU, including its officers, employees, and agents, from and against any and all liability, actions, debts, claims, and demands of every kind whatsoever, including, but not limited to, any claim for Third Party negligence and/or any present or future claim, loss, or liability for which my Child may be liable to any other person or to IU that arises out of my Child’s participation in the Program.
In the event of an accident or serious illness, I hereby authorize representatives of IU to obtain medical treatment and transport for my Child on my behalf. I waive my right to receive informed consent prior to such transportation or treatment. I hereby hold harmless and agree to indemnify IU from any claims, causes of action, damages and/or liabilities, arising out of or resulting from the medical treatment or transport. I further agree to accept full responsibility for any and all expenses, including medical expenses that may derive from any injuries to my Child that may occur during his/her participation in the Program.
This Agreement shall be governed by and construed under the laws of Indiana. Notwithstanding any other agreement that I have signed related to this Program that purports to establish the venue for any litigation arising from this Program, I agree that I will file no action against The Trustees of Indiana University or its officers, employees, and agents, whether based on this Agreement or in any way otherwise connected to this Program, in any court other than the Circuit Court of Monroe County, Indiana.
I understand and agree to all of the terms of this Agreement. I understand that I am giving up substantial rights (including my right to sue) and acknowledge that I am willingly signing this document. My signature on this document is intended to bind not only myself and my Child, but also the successors, heirs, representatives, administrators, and assigns of myself and my Child.

Photo, Video, and Audio Consent and Release Form

I (“Participant”) authorize The Trustees of Indiana University (“IU”), acting through its agents, employees, or representatives, to take photographs, video recordings, and/or audio recordings of me, including my name, my image, my likeness, my performance, and/or my voice (“Recordings”). I also grant IU an unlimited right to reproduce, use, exhibit, display, perform, broadcast, create derivative works from, and distribute the Recordings in any manner or media now existing or hereafter developed, in perpetuity, throughout the world. I agree that the Recordings may be used by IU, including its assigns and transferees, for any purpose, including but not limited to, marketing, advertising, publicity, or other promotional purposes. I agree that IU will have final editorial authority over the use of the Recordings, and I waive any right to inspect or approve of any future use of the Recordings. I acknowledge that I am not expecting to receive compensation for participating in the Recordings or for any future use of the Recordings.

I release and fully discharge IU, and its employees, agents, and representatives, from any claim, damages, or liability arising from or related to my participation in the Recordings or IU’s future use of the Recordings.

I have read this entire Consent and Release Form, I fully understand it, and I agree to be bound by it.

I understand that certain risks are inherent in travel and participation in the Field Trip. These risks may include, but are not limited to, such things as incidents related to travel to an urban setting, including transportation, driver error, adverse weather conditions, and exposure to theft and other criminal activity; sprains, broken bones, cuts, bruises, entrapment, temporary or permanent disability, and/or death; allergic reactions to food and drink items; other physical, mental, and emotional injury; other risks and dangers, whether known or unknown nor reasonably foreseeable; and the following additional risks: falling, traffic accident, tripping, fire or other emergency.
I understand that some drivers of vehicles in which I ride on this Field Trip, the owners, officers, employees, or agents of any attraction, enterprise or vendor of which I take part or participate during the Field Trip, the staff/employees of any hotel at which I stay, the staff/employees of any site I may visit, the other participants of the Field Trip (whether associated with my group or not), and other third parties (collectively, “Third Parties”), are not the agents or employees of IU and that dangers may be caused by the negligent or intentional act(s) or omissions of such Third Parties. I understand that IU is not responsible for any injuries or property damage that may be caused by the acts or omissions of such Third Parties.
I understand that my participation in this Field Trip is entirely voluntary and at my own risk. I fully understand the scope of the activities and the potential risks involved in the Field Trip. I agree to assume the risks of my participation in the Field Trip, including the risk of catastrophic injury or death.
I understand and agree that IU does not provide insurance to cover medical expenses for injuries that may be sustained by me or for damage to my personal property, and that IU strongly recommends that I carry my own health, medical, and property insurance for purposes of potential losses related to this Field Trip.
I fully understand that all IU policies and regulations, including those embodied in the Code of Student Rights, Responsibilities and Conduct, are in effect and apply to my behavior for the entire duration of the Field Trip. I understand that any violations of these policies and regulations may result in sanctions up to and including, in appropriate circumstances, referral to the Indiana University Police Department and/or the Office of Student Ethics for disciplinary action.

This Agreement shall be governed by and construed under the laws of Indiana. Notwithstanding any other agreement that I have signed related to this Field Trip that purports to establish the venue for any litigation arising from this Field Trip, I agree that I will file no action against IU or its officers, employees, and agents, whether based on this Agreement or in any way otherwise connected to this Field Trip, in any court other than a court in Marion County, Indiana.

Summer Camp Cancellation Policy

I understand and agree that all registrations for the IU Indianapolis Office of Undergraduate Admissions Summer Camps are subject to the following refund policy:

Cancellations made before May 1 will receive a full refund.
Cancellations made between May 1 and May 31 will receive a $200 refund.
No refunds will be issued after May 31 for any reason, including but not limited to scheduling conflicts, illness, travel delays, or personal circumstances.
Payments are non-transferable. After May 31, IU Indianapolis does not offer refunds for cancellations, no-shows, or early departures from the camp. In the event of camp cancellation by IU Indianapolis, alternative arrangements may be provided.
I have read this entire Agreement, fully understand it, and agree to this cancellation and refund policy. I represent and certify that my true age is at least 18 years old or, if I am under 18 years old on this date, my parent or legal guardian has also signed this Agreement.

Summer Camp Payment Policy

I understand and agree that all fees for the IU Indianapolis Admissions Summer Camp must be paid in full at the time of registration. A completed registration and full payment are required to secure a spot in the camp.
Payments are non-transferable. After May 31, IU Indianapolis does not offer refunds for cancellations, no-shows, or early departures from the camp. In the event of camp cancellation by IU Indianapolis, alternative arrangements may be provided.
